Registration

Dear Parent of Cheerleader / Football Player: 

This is a reminder that Registration will take place this Saturday,
July 25th and Sunday, July 26th, by appointment ONLY 
at the Madison Heights City Hall Basement
(300 W 13 Mile Road, Madison Heights, MI 48071). 
 Also, due to COVID-19 and the City of Madison Heights, before entering the City Hall, your temperature will be taken, you must wear a mask, (parent and child), and stay socially distant. 
PLEASE DO NOT BRING ANYONE OTHER THAN YOUR CHILD REGISTERING FOR FOOTBALL AND/OR CHEER.  IF YOU ARE FEELING SICK OR EXPERIENCING ANY SYMPTOMS OF COVID, PLEASE DO NOT COME.
 All participants will be fitted at registration and uniforms will be given out at a later date. As long as all of the following items are uploaded before registration, it should take no more than 30 minutes to fully register.

Current photo (if you have a photo uploaded from previous years, this photo must be replaced)
Birth Certificate
Physical (must be signed by both parent and physician)
Medical Release
Media Release
Athletic Consent Form
Waiver and Release Form
Concussion Info Sheet
Payment of $100.00 per child due at registration (Cash or check ONLY)
